PATHS – Providing Access to Health Services

Are there health needs holding you back?
Do you want to work?
Are you currently on a benefit?

If you have answered ‘yes’ to all of these questions then PATHS might be the service for you.


PATHS aims to remove, reduce or manage your health issues that prevent you from working. The goal is to help you address your unmet health needs which will enable you move into employment. PATHS is a joint service between Nelson Bays Primary Health and Work and Income.

Examples of what PATHS has been able to help with and/or fund:

  • CBT/counselling
  • Pain management
  • Navigation through services
  • Specialist appointments such as physiotherapy, orthopaedics, ophthalmology etc.
  • Some surgeries such as eye, shoulder, varicose veins etc.
  • Gym memberships
  • Travel assistance
  • Advocacy
  • GP appointments

You can be referred to PATHS by:

  • Your Work and Income Case Manager,
  • Your GP,
  • Other agencies you may be connected with.
